Unforuntately there are a few scams in this space and you need to watch out for them or you could get in trouble with your employer and possibly the state if you're opting for paid leave or disability. Here is what to look for:

1. Do you meet with a doctor over video? The Department of Labor requires a video evaluation for telehealth FMLA certification. Any service that certifies you through a questionnaire alone may not produce legally valid paperwork.

2. Are the providers licensed? Licensed in your state, and does the service tell you who you're seeing before you pay?

3. Does the provider actually evaluate your condition?
History, follow-up questions, clinical determination — or just read your intake form back to you and sign the WH-380?

4. Is their pricing honest? Some services quote one number then charge extra for employer verification, follow-ups, or "expedited processing, and some even opt you into a subscription model after you've already paid them money.If a service cuts corners on factors 1-3, it's your problem, not theirs. They already collected their fee. But your employer can reject the certification and your state can deny your paid leave claim.

5. Very few reviews or very poor reviews: A service claiming thousands of patients should have reviews to match

6. Opaque company identity: If you can't figure out who runs the service, that's intentional

Legitimate but expensive overkill if FMLA certification is all you need.
My experience
PlushCare is a legitimate telehealth platform. I give it high marks for general telehealth services. But for FMLA specifically, it's a different story. When I explored using PlushCare for FMLA certification, I discovered that FMLA is not a productized service. There's no "FMLA certification" button. Instead, you book a general appointment, explain to the doctor that you need FMLA paperwork, and hope they're willing and able to complete it.The problem is that completing FMLA certification properly,  especially the WH-380 form requires specific expertise.

The doctor needs to understand what "serious health condition" means under FMLA (it's a legal definition, not just a medical one). They need to know how to document "essential functions" they've never observed. They need to estimate leave duration in a way that's clinically supportable and doesn't invite employer pushback.A general telehealth doctor doing a 15-minute video visit may not have this expertise. Some PlushCare doctors have reported requiring multiple appointments and a review of existing medical records before they'll complete FMLA paperwork. That means multiple $129 visits (or multiple copays), plus the $19.99/month membership, plus the time and stress of coordinating records.

MyFMLA is a legitimate service, and I want to be clear about that. Dr. Aaron Sartin and Dr. Sally Sartin are real, named, board-certified family medicine physicians. You can verify them. The evaluation process is professional, and their Trustpilot reviews are genuinely excellent, 96% five-star is hard to fake.

I booked a next-day appointment for an intermittent FMLA evaluation. The 20-minute video call is thorough and professional. Dr. Sartin asks detailed questions about symptoms, their frequency, how they affected your ability to work, and what kind of schedule flexibility you need.

Dr. Sartin clearly understands the WH-380 form, what employers look for, and what other programs you are likely eligble to apply for. They are knowledable, professional, and reliable.

So why isn't MyFMLA my number one?Three reasons.

1. First, the price: If you need both FMLA and short-term disability documentation, MyFMLA charges $249.99, which was one of the most expensive services on this list.

2. Second, the scale: MyFMLA has two doctors. Two. For their entire patient volume across all three of their domains. Two doctors also means limited availability, if both Drs. Sartin are booked, you wait.

Third, the refund policy: Once your certification is issued, there are no refunds.

The quality is there and this is a great alternative.

You could book a $99 urgent care visit and ask the provider to fill out your WH-380, but you're hoping a general telehealth doctor will take on paperwork that isn't part of their standard workflow and there may not be enough time for a proper evaluation and form completion. If your employer pushes back, there's no built-in support for follow-up or verification. A legitimate platform for seeing a doctor when you're sick, not for getting FMLA paperwork done.

FMLADocs does their evaluation with no visit, which as far as I know is not compliant with DOL and could get you in hot water. Additionally, I had to dig to figure out that they charge you per form which makes the price really expensive as you get more forms. Their third-party reviews are not good either. The lack of transparency and concerning reviews makes it impossible to recommend over other services.
My experience
FMLADocs is not a service I would personally use. The main drawback is they operate without requiring a video visit. From my research this is not a valid telehealth visit for FMLA, Paid Leave, or Disability and would not meet federal or state requirements if scrutinized.

Here were the issues I had:

1. No video visit: For the reasons I mentioned above this is an automatic deal breaker.

2. Expensive Pricing: No pricing on the homepage. You finally see the price when you get to the checkout at $129, but when I proped futher, they charge $129 per form, so this can be a lot more expensive than other services.

3. Reviews: The reviews on TrustPilot are all one stars with customers citing poor experiences, scam, and other negative feedbck. This was a red flag. It either means the service is very new and trying to work out kinks or something else.

Overall would not recommend above some of the other services.

My experience
FMLANow's $49 price tag is what drew me in. But after reading their terms and conditions, I discovered auto-renewing subscription fees that aren't disclosed upfront. One of their Trustpilot reviewers called it a 'bait and switch.' and this shows up in most of their complaints. Their TrustPilot reviews are abysmal with many users calling it a "scam" and accusing the service of wracking up hundreds of dollars in expenses. Additionally, the operate in a very narrow scope of mental health. For a service where your job protection is at stake, the misleading fees and poor reviews is not something I'd gabmel on..
